Kybella Before and After: Swelling, Sessions, and What the Result Really Looks Like

Every Kybella before and after photo skips the same week: the one right after treatment, when the chin swells into what patients call the bullfrog. That week is the price of a permanent result. This guide shows the whole arc, session by session, so you know what you are signing up for.

Kybella is the only injectable approved to permanently destroy fat, and it is aimed at one place: the pad under the chin. The before and after photos are genuinely impressive, a soft, rounded submental area becoming a defined jawline over a few months. What the photos never show is the week after each injection, when the area swells dramatically, and the fact that the result takes two to four sessions to build. This guide covers the whole arc.

What Kybella is

Kybella is deoxycholic acid, a synthetic version of a molecule your body already makes to digest fat. Injected in a grid across the fat pad under the chin, it breaks down the membranes of fat cells in the treated area. The body clears the cell debris over the following weeks. Destroyed cells do not regenerate, so the fat that is gone stays gone as long as your weight is stable.

It is approved for moderate to severe submental fullness and is used off-label in small pockets elsewhere, like the jowls or bra line, by experienced injectors.

The before and after, session by session

PointWhat you see
BeforeA soft pad under the chin that blurs the line between chin and neck, often regardless of weight
Treatment day20 to 30 tiny injections over about 15 minutes; burning and stinging for a few minutes; immediate swelling
Days 1 to 5Significant swelling, sometimes doubling the apparent size of the area, with numbness and tenderness. The bullfrog phase.
Days 5 to 14Swelling subsides steadily; numbness lingers; the area may feel firm
Week 4 to 6Firmness resolves; a modest reduction in fullness is visible. Second session is typically here.
After session 2, month 3Clear improvement; the chin-to-neck angle is sharper
After session 3 or 4, month 4 to 6Full result. A defined jawline where there was a pad. Most people finish here.

The swelling is the story. It is not a side effect in the usual sense; it is the treatment working, as the fat cells break down and the body responds. It peaks at day two or three, is very visible for the first five days, and can take up to two weeks to fully settle. Plan each session for a stretch where you can work from home or wear a scarf, and do not schedule one in the two weeks before a wedding. Numbness in the area is normal and can last a few weeks.

Each session removes a portion. One session produces a modest change most people can see in careful photos; two produces a clear one; three to four produces the result in the marketing. Sessions are spaced at least a month apart. A minority of people with mild fullness are done in two; a minority with substantial fullness need six.

What a realistic result looks like

At month four to six after the final session, the fat pad is largely gone and the jawline reads as a line rather than a curve. The skin usually contracts to fit, especially in people under 50 with reasonable elasticity. The result is permanent for the treated cells. What it is not is a neck lift: loose skin, platysmal bands in the neck, or a recessed chin are not fat problems, and Kybella does not address them. An honest assessment before you start separates fat from skin, and if the answer is mostly skin, the recommendation should be tightening or surgery rather than injections.

Getting through the swollen week

A few things make the bullfrog phase easier, and clinics that do a lot of Kybella hand them out as a checklist. Ice the area for the first two days in short intervals; it takes the edge off both swelling and tenderness. Sleep with your head elevated on an extra pillow for the first few nights. Skip alcohol, salty food, and hard exercise for two or three days, all of which make swelling worse. Over-the-counter pain relief is fine if your injector approves it; many prefer acetaminophen over anti-inflammatories in the first day. A soft scarf or a high collar covers the area for anyone who needs to be seen. And take a photo on day one, day seven, and day fourteen, because the swelling is dramatic enough that you will not believe how quickly it resolved unless you can look back.

Numbness deserves its own mention. The area often feels wooden for two to four weeks, and a small number of people notice it for longer. It is expected, it fades, and it is not a sign of anything wrong. Difficulty swallowing or a smile that looks uneven are different, uncommon, and worth a call to the clinic the day you notice them.

What it costs

Kybella is priced per vial, and a session uses one to three vials depending on the size of the pad, so a session commonly runs $600 to $1,500. A full course of two to four sessions typically totals $1,500 to $4,500. That is more than a CoolSculpting series for the same area in many clinics and less than submental liposuction. Ask for the estimated vial count per session and the total for the course, in writing, before the first injection.

Kybella, CoolSculpting, or liposuction

All three permanently remove submental fat. They differ in experience and speed.

  • Kybella is precise, sculptable along the jawline, and involves a swollen week per session. Best for moderate, well-defined pads and for people who want the injector’s hands shaping the result.
  • CoolSculpting’s chin applicator freezes the pad with less swelling and more soreness, over one or two sessions. Lower drama, less precision at the edges.
  • Submental liposuction removes far more fat in one procedure, with a week of compression garment and surgical recovery. The right choice for large pads or when you want it done in one step, and often paired with skin tightening.

Our double chin guide walks through the fat-versus-skin decision that determines which of these fits.

One more comparison point that matters in practice: timing. A Kybella course takes four to six months from first injection to final result because of the spacing between sessions and the slow clearing of fat. CoolSculpting is similar. Liposuction delivers its result in the time it takes swelling to settle, roughly four to six weeks. If there is a date you are working toward, count backward from it before choosing.

Choosing an injector

Kybella placed too close to the jawbone can affect the nerve that moves the lower lip, producing a temporary crooked smile that lasts weeks. It is uncommon and it is a placement error. Ask how many Kybella treatments the injector performs, whether they mark the treatment zone against anatomical landmarks before injecting, and how they handle a patient whose fullness turns out to be skin. Experience with this specific product matters more than general injecting experience.
