Silicone or saline?
Silicone implants feel more like natural breast tissue and are what most patients choose today. Saline implants cost less, use a smaller incision, and make rupture obvious immediately since the body absorbs the saline harmlessly. Silicone rupture can be silent, which is why the FDA recommends periodic MRI or ultrasound screening for silicone implants. Both are FDA approved and both are safe options. The decision is genuinely personal.
What are the real risks?
The most common is capsular contracture, where scar tissue tightens around the implant and firms or distorts the breast, which affects a meaningful minority of patients and may require revision surgery. Others include rupture, changes in nipple sensation that are sometimes permanent, rippling, and asymmetry. There is also a rare lymphoma called BIA-ALCL associated primarily with textured implants, which is why textured implants have largely left the US market. Discuss all of this at consultation, and ask the surgeon what their own revision rate is.
Will I be able to breastfeed afterward?
Most patients can, particularly when the incision is placed in the fold beneath the breast or the armpit rather than around the areola. Incisions around the areola carry somewhat higher risk of affecting milk ducts and nipple sensation. Tell your surgeon if future breastfeeding matters to you, because it should influence the incision choice.
