The upper third of the face is where a misdiagnosis costs the most. Thousands of people are quoted for eyelid surgery every year when their actual problem is a brow that has slid downward, pressing spare skin onto the lids and recruiting the forehead muscle into a permanent, exhausting lift. A brow lift treats that cause directly, which is why surgeons assess the brow before they ever discuss the eyelids.
What the operation involves
The modern standard is the endoscopic brow lift. Through a few small incisions hidden behind the hairline, the surgeon releases the tissue holding the brow down, repositions it to its natural height, and secures it with small anchors or sutures while it heals into place. The muscles that pull the brow inward and downward, the ones that carve frown lines, are often weakened at the same time.
It runs one to two hours under sedation or general anesthesia. A temporal lift, confined to the outer brow, is a smaller version for the common pattern where only the tail of the brow has fallen, taking the upper lid’s outer hood with it.
The brow or the eyelid, and how to tell
Look in a mirror and place a finger at the middle of one eyebrow. Lift it gently to the level of the bony rim above your eye. If the hooding over your lid largely disappears, the brow is the problem, and blepharoplasty alone would under-deliver. If the brow already sits at the rim and the fold remains, the excess is lid skin and blepharoplasty is the operation.
Many people past fifty have some of both, which is why the combination is among the most common pairings in facial surgery. Getting this diagnosis right is worth an in-person consultation with someone who performs both operations and has no reason to steer you to either.
What it costs
Nationally, $4,000 to $9,000 all in. A temporal lift runs lower, often $3,000 to $5,000. Adding blepharoplasty to a brow lift typically brings the combined total to $8,000 to $14,000, below the price of staging them, with one anesthesia and one recovery.
A Botox brow lift, for comparison, costs a few hundred dollars per session and buys a subtle effect measured in months. It is the right tool early, and the arithmetic stops favoring it once you are maintaining it indefinitely against a descent it cannot correct.
Recovery
Days 1 to 3: forehead tightness, swelling that drifts down toward the eyes
Days 4 to 7: bruising around the eyes at its most visible, then fading
Week 1: most sutures or clips out, desk work resumes for most patients
Week 2: presentable in public, makeup covers what remains
Weeks 3 to 4: light exercise, numbness on the scalp beginning its slow fade
Months 2 to 6: full sensation returns gradually, final brow position settles
Temporary numbness or itching across the scalp is expected and can persist for a few months.
Whether it is the right operation
The tell is the forehead itself. If your forehead lines are deep because you hold your brows up all day to see clearly or to avoid looking tired, the muscle is compensating for a structural descent, and relaxing those lines with Botox will actually make the heaviness worse. That pattern is the textbook brow lift candidate.
Browse plastic surgeons in your city and ask in consultation whether your upper lids, your brow position, or both are driving what you see.
Why people choose Brow Lift
Lifts the brow itself rather than trimming eyelid skin, treating the actual cause of many hooded eyes
Opens the upper eye area and relieves the habit of holding the forehead raised
Softens a heavy, stern, or tired resting expression
Frequently combined with eyelid surgery for a balanced upper-face result
Are you a good candidate?
People whose brows sit low or press onto the upper eyelids, who raise their eyebrows constantly to see or look alert, or whose resting expression reads as tired or angry

Brow Lift guides & cost breakdowns
Honest, in-depth reading to help you compare options and budget with confidence.
Nationally, $4,000 to $9,000 including surgeon, anesthesia, and facility. Endoscopic lifts through small incisions in the hairline occupy most of that range. A limited temporal lift addressing only the outer brow runs lower. Combining a brow lift with eyelid surgery is common and costs less than staging the two operations separately.
Do I need a brow lift or eyelid surgery?
This is the central question of the upper face, and plenty of people get the wrong operation. If the fold hanging over your eye is excess lid skin while the brow sits at its normal height, blepharoplasty is the answer. If the brow itself has descended and is pushing the lid down, removing lid skin leaves the heaviness and can pull the brow lower. The mirror test helps, and a surgeon who evaluates both routinely gives the honest answer. Many patients benefit from both together.
Will a brow lift make me look surprised?
The startled look comes from overcorrection, lifting the brow past its natural position, and it was a hazard of older techniques that pulled hard on the forehead. Modern endoscopic lifts reposition the brow to where it sat in your thirties and secure it there. Reviewing a surgeon's results specifically for natural brow position is the practical safeguard.
How does a surgical brow lift compare to Botox or a thread lift?
A Botox brow lift relaxes the muscles that pull the brow down, buying a couple of millimeters for three to four months, and it is a reasonable preview or maintenance tool. Threads lift modestly and fade within a year or so. Surgery repositions the brow by a genuine amount and holds for a decade or more. The non-surgical options serve early or mild descent, and they cannot substitute for surgery once the brow truly sits low.
Where are the incisions?
Endoscopic lifts use three to five incisions under an inch long, hidden behind the hairline, through which the surgeon releases and repositions the brow with a camera. Temporal lifts use small incisions at the temples. Older coronal techniques ran an incision across the top of the head and are now uncommon. Hairline position matters to the plan, so mention any concern about a high forehead in consultation.

1.Brow Lift, American Society of Plastic SurgeonsConfirms a brow lift reduces horizontal forehead wrinkles and frown lines and raises sagging brows that hood the upper eyelids.
2.Brow Lift Recovery, American Society of Plastic SurgeonsConfirms initial wound healing takes about 10 to 14 days with return to work and normal activity in that window, and that swelling continues to resolve over several weeks, matching the page's recovery timeline.
3.Brow Lift Risks and Safety Information, American Society of Plastic SurgeonsLists numbness, altered skin sensation, and intense itching, plus hair loss around the incisions, as recognized brow lift risks, matching the page's note that temporary numbness or itching across the scalp is expected.
4.Endoscopic Brow Lift, StatPearls, National Library of Medicine (National Institutes of Health)Describes the small incisions placed behind the hairline, general anesthesia or intravenous sedation as the recommended approach, and resection or ablation of the corrugator and procerus muscles, which carve frown lines, as part of the procedure.
