Implant or genioplasty?

It suits a mild to moderate recession where the chin only needs to come forward. A sliding genioplasty cuts the lower edge of the chin bone, moves it forward, down or sideways, and fixes it with a small plate: one to two hours, done by a plastic or oral-maxillofacial surgeon, and it can lengthen a short chin or correct asymmetry as well as project it. The 2025 systematic review of 1,126 patients found implants suit mild to moderate deficiency and genioplasty suits severe deficiency and complex three-dimensional problems, with high satisfaction for both and a trend toward higher satisfaction after genioplasty.

How much does a chin implant cost?

A silicone chin implant runs $4,000 to $8,000 in the US including surgeon, anesthesia and facility; a sliding genioplasty $7,000 to $12,000 because it is longer and needs plating. Combining with rhinoplasty or chin liposuction in one operation is common and cheaper than doing them separately. Canadian prices track similar figures in dollars. Is chin filler a good preview?

Yes, and it is how most people arrive at surgery. One to two syringes of firm filler on the chin shows you the effect of projection on your whole profile for twelve to eighteen months, reversibly. If you love it and would rather stop paying yearly, an implant makes it permanent. Filler cannot lengthen a very short chin or correct asymmetry convincingly; those are genioplasty problems from the start.

What are the risks?

The ASPS list covers both methods: anesthesia risks, bleeding, hematoma, infection, asymmetry, numbness of the chin and lower lip from the mental nerve, poor healing, prolonged swelling, a result you are not happy with and the possibility of revision. Implants add displacement, capsular contracture (thick scar around the implant) and, over years, some erosion of the bone beneath. The review's numbers: implant infection ranged from 0% to 23.8% across studies and persistent sensory change was 9.4%; genioplasty infection was 0% to 5.3%, temporary numbness was very common, and numbness lasting beyond a year affected 7.4% to 12.5%. What is recovery like?

A chin strap for a week, swelling and bruising along the jaw for two weeks, one to two weeks off work, and a soft diet plus mouth rinses for a week if the incision is inside the mouth. Numbness of the chin and lower lip is expected for weeks to months, longer after genioplasty. Most swelling is gone by four to six weeks and the final shape is clear at three months. Implants can be felt but not seen once healed; genioplasty leaves nothing to feel.

Does a chin implant look natural?

When the size and shape are chosen for your face, yes; the change is in proportion rather than a feature people can point to, and the usual comments are that the nose looks smaller or the neck tighter. Overprojection and a too-wide implant are the recognizable failures, and they are a surgeon's judgment error, not a property of the implant. Can it be combined with other surgery?

Often. Surgeons regularly pair chin augmentation with rhinoplasty, because nose and chin are judged together in profile, and with chin liposuction or a neck lift, because projecting the chin tightens the line under it.
