Is it safe, and does it affect mammograms?

The safety questions were answered largely in reconstruction, where fat grafting has been used for decades. Landmark studies of 137 patients followed 7.6 years and of 1,024 breasts found no effect on breast cancer recurrence, the ASPS concluded in 2009 that fat grafting was not associated with higher malignancy rates, and the FDA in 2017 classed autologous fat grafts as allowed without premarket approval. Fat that does not survive can form small oil cysts or calcifications, which can show on a mammogram; radiologists can now consistently distinguish these from the calcifications of cancer, but they may call for additional imaging. Tell every radiologist you have had fat grafting, and have a baseline mammogram before surgery if you are of screening age.

What are the risks?

Bleeding and bruising at both the donor and breast sites, infection, fat necrosis (fat that dies and forms firm lumps or oil cysts), fat that shifts or is absorbed unevenly, asymmetry, calcifications, and the liposuction risks of contour irregularity and numbness at the donor site. Fat embolism is a rare, potentially fatal complication reported across fat grafting; it correlates with technique, which is one more reason to choose a surgeon who does this routinely. There is no implant, so there is no rupture, contracture, or implant-associated lymphoma to think about.

What is recovery like?

Plan a week off work. The liposuction areas are bruised and wear compression for four to six weeks; the breasts are swollen and tender for two to three weeks, with swelling settling over three to six months, which is when the result is judged. No pressure on the breasts for the first weeks: a soft bra, no sleeping face down, no underwire. Fat that has survived at six months is there for good at a stable weight.

Fat transfer or implants?

Implants give a predictable, adjustable, larger increase and a rounder upper breast, and come with a device that the FDA says is not a lifetime product and will need monitoring and eventually replacement. Fat transfer gives a smaller, softer, more natural change, slimmer donor areas, and nothing to replace, at the price of less predictability and the need for enough fat.
