Face Fat & Chubby Cheeks: treatment options

How do you lose face fat, and when is a procedure the answer?

A round face is usually overall weight, wide jaw muscles, a buccal fat pad, fullness under the chin, or simply bone, and each has a different answer. Weight loss slims the whole face. Masseter Botox narrows a jaw that is muscle. Buccal fat removal hollows the cheeks permanently and is the procedure most often regretted. Honest diagnosis comes first.

A round face has at least five causes and most people searching for how to lose face fat are treating the wrong one. It can be overall body fat, which the face shares. It can be the masseter muscles at the back of the jaw, enlarged by clenching and grinding into a square lower face. It can be the buccal fat pad in the mid-cheek. It can be fat under the chin. Or it can be bone: wide cheekbones and a wide jaw that no treatment alters. Sort out which you have, and the answer follows.

Start with weight

If you are carrying extra weight on your body, your face is carrying it too, and losing it is the only thing that slims the face as a whole. No exercise, cream, tool or massage removes fat from one spot. A supervised weight loss program takes it off everywhere, and the face follows, usually early. Lose it slowly, around a pound or two a week, because fast loss thins the face faster than the skin can tighten and produces the hollow, older look people now call Ozempic face. If your weight is normal and your face is still full, read on.

A square jaw: muscle, not fat

Clench your teeth and press the back corners of your jaw. A firm block that bulges under your fingers is the masseter, and if yours are large the width of your lower face is muscle. Masseter Botox relaxes it; the muscle thins by about a third over three months and the lower face narrows. The effect lasts three to four months at first, and about half of patients need a second round within four to seven months, after which results tend to hold longer. Side effects are mostly temporary: weaker chewing, occasionally a changed smile or a sunken look at the cheek. It does nothing for fat, so if the jaw is soft rather than firm, this is not your treatment.

Full mid-cheeks: the buccal fat pad

The buccal fat pad sits in the middle third of the cheek, under the cheekbone. Buccal fat removal takes it out through a small incision inside the mouth, permanently, leaving a hollow under the cheekbone. It is the procedure most likely to be regretted, and plastic surgeons say so themselves: for every five patients who ask, about one really needs it. Facial fat shrinks with age anyway, so a face hollowed in its twenties can look gaunt and older than it is twenty years on. It is for a genuinely full lower cheek on a face that will stay full, never for a thin or narrow face, and the long-term effect on aging has not been properly studied. The pad also sits next to the facial nerve and the parotid duct; in the published series about 8% of patients had a complication, mostly minor.

Fullness under the chin

A pocket of fat under the chin rounds the whole face from the front. Kybella dissolves it permanently over a few sessions; it is approved for the chin only, and its label says use anywhere else is not established and not recommended. Chin liposuction removes a defined pocket in one procedure with a sharper jawline, best on firm skin. If the skin under the chin is loose rather than full, neither is the answer and you are on our double chin and jowls pages.

When nothing on this page applies

Wide cheekbones and a broad jaw are bone, and a round face at a healthy weight with soft jaw muscles and a modest buccal pad is simply your face. A good consultation will say so. Puffiness that varies day to day is fluid, not fat. Puffiness that is new, persistent or one-sided belongs with a doctor first.

How to choose a provider

Ask the provider to name which of the five causes they see in your face, and why. Be wary of a clinic that offers buccal fat removal to a slim person in their twenties, or Kybella for the cheeks. For masseter Botox, ask how many units they plan and how they judge the dose. For buccal fat removal, a board-certified plastic or facial plastic surgeon who will show you patients ten years after surgery, not ten weeks.

Browse the clinics and surgeons below. Bring a photo of yourself at a lower weight if you have one; it shows a provider how much of the fullness is weight and how much is structure.

Your options

Treatments for face fat & chubby cheeks, compared

Best-first, with honest notes on when each is the right call.

Treatment Typical cost Downtime When it's the right call
Masseter Botox $400–$800 per session None. Possible mild soreness when chewing for a few days If the width is at the back of the jaw and you clench or grind, it is muscle. Botox thins the masseter by about a third over three months and the lower face narrows. Wears off in three to four months at first, and about half of people need a second round within four to seven months.
Buccal Fat Removal $2,500–$6,000 per procedure 3 to 7 days of visible swelling, final result at 3 to 6 months Removes the fat pad from the mid-cheek for a permanent hollow under the cheekbone. Only for a genuinely full lower cheek on a face that will stay full; on a thin or young face it ages badly, and leading surgeons say only one in five who ask for it needs it.
Kybella $600–$1,500 per session 1–2 weeks of significant swelling under the chin Dissolves fat under the chin permanently over a few sessions. Approved for the chin only; use on the cheeks or jowls is not established and not recommended.
Chin Liposuction $2,500–$6,000 per procedure 2 to 5 days off work, a chin strap for a week, swelling and bruising settling over 2 weeks, final contour at 3 months For a defined pocket of fat under the chin and along the jaw in someone with firm skin. One procedure, lasting result, sharper jawline.
Medical Weight Loss (GLP-1) $250–$1,500 per month None. Mild nausea or fatigue possible in the first weeks of dose escalation When the whole body carries extra weight, the face carries it too, and no local procedure fixes that. A supervised program takes it off everywhere, the face included; lose it slowly so the skin keeps up.

FAQ

Face Fat & Chubby Cheeks: common questions

Can I lose fat from my face specifically?

No. Fat comes off the body as a whole, and the face is included. If you are carrying extra weight overall, losing it is the thing that will slim your face, and no cream, exercise or massage will do it locally. If your weight is normal and the face is still full, the fullness is muscle, a fat pad, the chin, or bone, and that is what a consultation should sort out.

How do I tell muscle from fat?

Clench your teeth and feel the back corners of your jaw. If a firm block bulges under your fingers, your masseter muscles are large, and the width of your lower face is muscle. That responds to Botox, not to any fat procedure. Full mid-cheeks that soften when you suck them in are the buccal pad. Fullness under the chin that you can pinch is submental fat.

Should I get buccal fat removal?

Probably not, and leading plastic surgeons say so: for every five people who ask, about one is a real candidate. The fat pad is removed for good, and facial fat keeps shrinking with age, so a face hollowed at 25 can look gaunt and older than it is at 45. The procedure is for a face that is genuinely full in the lower cheek and will stay that way. It is not for a thin or narrow face, and it is not a weight loss shortcut. The long-term effect on aging has not been properly studied.

Does Kybella work on cheeks or jowls?

It is approved for fat under the chin. Its label states that use on fat anywhere else has not been established and is not recommended. The cheek also contains nerves and the parotid duct, which is one reason surgeons treat it carefully.

Could it be bloating rather than fat?

A face that is puffier some mornings than others, or after salt, alcohol or a bad night, is fluid, not fat, and no procedure addresses it. Puffiness that is new, persistent or one-sided is a reason to see a doctor before a clinic.

  1. 1.Buccal Fat Removal, American Society of Plastic SurgeonsThat buccal fat removal thins the cheeks by removing the buccal fat pad, that the size of the pad varies between people and between the two sides, that it is for people whose face feels too full or chubby, and that it is typically not performed in people with thin, narrow faces because removing the fat may make them look gaunt with age.
  2. 2.Facial fat is precious: Why your plastic surgeon shouldn't be too aggressive with buccal fat removal, American Society of Plastic SurgeonsThat fat in the face is precious and the one place you do not want to lose a lot of it, that removing the buccal pad may look good now but can leave the face overly hollow and prematurely aged twenty years on, that for every five patients who ask for it only one really needs it, that demand has grown among patients in their twenties and thirties who may not have excess buccal fat, that fullness often has other causes, that an overactive masseter is treated with Botox and that jawline augmentation or neck tightening may address the concern instead, and that buccal fat should be removed only for excess fullness in the lower front face with a specific reason.
  3. 3.Buccal fat pad removal to improve facial aesthetics: an established technique?, Medicina Oral, Patologia Oral y Cirugia Bucal (PMC, National Institutes of Health)That the buccal fat pad is a capsule-bound fat structure in the middle third of the cheek close to the facial nerve and parotid duct, that removal is done for a rounded face or excessive cheeks, that the only absolute contraindication identified is hemifacial atrophy, that complications in the reviewed series included asymmetry, trismus and one near-fatal hemorrhage at a rate of 8.45%, that the amount removed is often undescribed and excess removal may give an unfavorable outcome, that follow-up in the literature was short, and that the long-term effect on facial aging is not established.
  4. 4.KYBELLA (deoxycholic acid) injection: Prescribing Information, U.S. Food and Drug Administration (Drugs@FDA label, 2015)That Kybella is indicated for improvement in the appearance of moderate to severe fullness associated with submental fat in adults, and that its safe and effective use for subcutaneous fat outside the submental region has not been established and is not recommended.
  5. 5.The Role of Botulinum Toxin for Masseter Muscle Hypertrophy: A Comprehensive Review, Toxins (PMC, National Institutes of Health)That botulinum toxin reduces masseter muscle volume, with thickness reduced by an average of 31% at three months on ultrasound in a series of 1,021 patients; that the effect lasts about three to four months and half of patients needed a second injection after four to seven months; that reduced chewing strength, a changed smile, paradoxical bulging and sunken cheeks are among the reported side effects; that there is no consensus on dose; and that repeated injection at short intervals may cause a permanent decrease in muscle thickness.
  6. 6.Liposuction, American Society of Plastic SurgeonsThat liposuction can reduce localized fat deposits of the face, cheeks, chin and neck, that firm elastic skin gives better results, and that it is not a treatment for obesity or a substitute for diet and exercise.
  7. 7.'Ozempic Face': What It Is and How To Avoid It, Cleveland Clinic Health EssentialsThat rapid weight loss thins the face, producing sunken eyes, new wrinkles, thinner lips and sagging skin from lost fat under the skin, and that losing one to two pounds a week limits it, backing the page's advice that weight loss slims the face and that losing it slowly lets the skin keep up.
