What is lipo 360, exactly?

Liposuction of the full circumference of the torso in one operation: upper and lower abdomen, flanks (love handles), waist, and the lower and often mid back, sometimes the bra roll. It is not a different technique from liposuction; it is a different plan. Treating the front alone can leave a waist that looks finished from ahead and unfinished from behind, so the surgeon contours the whole ring so the shape reads the same from every angle. Many patients add fat transfer to the buttocks (a BBL) or breasts in the same operation using the fat removed.

How much does lipo 360 cost?

In the US, $7,000 to $15,000 all-in for the surgeon, anesthesia and facility, with most quotes landing between $8,000 and $12,000. High-definition or etching work adds $2,000 to $5,000 because it takes longer and demands more skill. Adding a BBL usually brings the package to $12,000 to $20,000. Compression garments, lymphatic massage and follow-ups may or may not be included; ask. Prices under $5,000 usually mean a non-plastic-surgeon, a non-accredited facility, or a small number of areas being called 360.

Who is a good candidate?

Someone within about 30 pounds of a stable weight, with fat that pinches (under the skin) rather than a hard, rounded abdomen (fat around the organs, which liposuction cannot reach), and with skin firm enough to retract over the new shape. Skin is the deciding factor: after pregnancies or large weight loss, loose skin gets looser once the fat under it is gone, and the honest recommendation becomes a tummy tuck, or a tuck combined with lipo of the flanks and back. High-definition etching is for people who are already lean and athletic; on a softer body it produces lines that look drawn on. Lipo 360 is not a weight loss method.

How much fat can be removed safely?

The American Society of Plastic Surgeons defines large-volume liposuction as more than five liters removed and recommends it be done where the patient can be monitored overnight. In an ASPS analysis of more than 4,500 patients, the overall complication rate was 1.5 percent, rising to 3.7 percent above five liters, almost entirely from fluid collections (seromas) that needed draining, and the safe volume depended on BMI. A lipo 360 on a slim patient might remove one to two liters; on a larger patient three to five. A surgeon who quotes volume before examining you, or plans to take more than five liters in an office setting, is a reason to look elsewhere.

What is the difference between lipo 360, HD lipo and VASER?

Lipo 360 describes the areas: the full circumference of the torso. High-definition (HD or 4D) lipo describes the goal: sculpting closer to the muscle so the abdominal lines and obliques show, usually on an already lean patient. VASER, laser lipo, SmartLipo, power-assisted and BodyTite describe the tool used to loosen or heat the fat before it is suctioned. A surgeon can do a 360 with any tool and with or without an HD approach. The tool matters less than the surgeon's judgment about your skin and how much to take; the marketing says otherwise because devices are what clinics buy.

What is recovery like?

Expect one to two weeks off desk work and four to six weeks off heavy exercise. A compression garment is worn almost continuously for about six weeks. Bruising and swelling peak in the first week and most of it is gone by week four, but the final shape takes three to six months as swelling settles and skin retracts. Many surgeons prescribe lymphatic drainage massage in the first weeks to reduce swelling and lumpiness; the evidence is modest but it is standard. Numbness in the treated skin can last months and occasionally persists.

What are the risks?

From the ASPS list: contour irregularities and asymmetry (the most common reason for revision), fluid collections, persistent numbness, infection, poor wound healing, irregular pigmentation, rippling or loose skin, worsening cellulite, and rarely damage to deeper structures including abdominal organs, blood clots, and fat embolism. Energy-assisted techniques add the risk of burns. The circumferential approach means more surface area and more fluid shifts than a single-area lipo, which is why the facility, the anesthesia provider and the volume plan matter more here than in a chin lipo. The FDA's 2022 to 2023 communications on the Renuvion device, which some surgeons use under the skin after lipo to tighten, clear it only after liposuction for body contouring, not for skin tightening on its own.

Lipo 360 or a tummy tuck?

Lipo removes fat; a tummy tuck removes skin and repairs separated abdominal muscles. If your skin is firm and your muscles are intact, lipo 360 gives a smaller scar and a faster recovery. If you have loose or stretch-marked skin, a hanging lower belly, or muscle separation after pregnancy, a tuck is the operation that fixes it, and most surgeons combine it with lipo of the flanks and back so the result is 360 anyway.
