If future breastfeeding matters to you, say so clearly at consultation, since technique choices can preserve more of the ductal system. And if you plan to have children soon, waiting is generally the better decision anyway.
Almost never as a cosmetic lift. Breast reduction, which removes tissue as well as lifting, is sometimes covered when documented neck, back, or shoulder symptoms meet a carrier's criteria. If you have both sagging and significant volume causing physical symptoms, ask the surgeon's office to explore reduction coverage rather than assuming the whole thing is out of pocket.
