Semaglutide vs. Tirzepatide: Results, Cost & How to Choose

If you’re considering medical weight loss in 2026, the decision usually comes down to two drugs: semaglutide (Wegovy/Ozempic) and tirzepatide (Zepbound/Mounjaro). Both are weekly injections. Both produce weight loss that no medication before them came close to. Here’s how they actually differ.

Results: tirzepatide wins the averages

For years the two drugs were compared across separate trials. The 2025 SURMOUNT-5 head-to-head ended the guesswork: tirzepatide patients lost meaningfully more weight than semaglutide patients, roughly 20% versus 14% on average.

Two caveats keep this honest. Averages aren’t individuals: plenty of patients get excellent results on semaglutide, and a minority respond minimally to either. And more medication effect isn’t automatically the goal. A patient targeting 25 pounds doesn’t need the category’s maximum horsepower.

Cost: closer than the list prices suggest

List prices are misleading in both directions. Both manufacturers now sell direct to cash-pay patients. Zepbound single-dose vials run roughly $349 to $499 per month, and Wegovy’s direct pharmacy pricing landed around $499 per month. Med spa and clinic programs bundle supervision, labs, and dose management, historically in the $250 to $600 monthly range depending on formulation.

The wildcard is insurance: semaglutide’s longer time on market means broader coverage. If your plan covers Wegovy, semaglutide is probably your cheapest path. If you’re cash-pay, compare the direct-vial programs before assuming anything.

One sourcing note that matters in 2026: the FDA-declared shortages that permitted mass compounding are over, and the rules on compounded GLP-1s have tightened sharply. Whatever a provider offers, ask exactly what’s dispensed and where it’s sourced.

Side effects: same family, similar toll

Nausea during dose escalation is the tax both drugs charge, alongside constipation, fatigue, and reflux. Some patients report tirzepatide feels gentler, possibly the GIP effect, but this varies person to person. The serious-but-rare risks (pancreatitis, gallbladder issues) and the thyroid-cancer-history exclusions are identical.

Both also share the cosmetic side effect nobody mentions until it’s in the mirror: rapid weight loss deflates your face. “Ozempic face” applies to every GLP-1, and it’s why many patients pair the end of their weight-loss phase with dermal fillers or facial volume restoration.

How to actually choose

Pick tirzepatide if: you want the strongest average results, semaglutide underdelivered for you, or the direct-vial cash price beats your semaglutide options.

Pick semaglutide if: your insurance covers Wegovy, you value the longest safety track record in the category, or your provider’s program prices it meaningfully lower.

Either way: a legitimate program starts with labs and medical history, escalates your dose patiently, and talks maintenance strategy before your first injection. A provider who skips straight to the syringe is the wrong provider.
